I understand that physical activity and exercise may involve risks including, but not limited to, muscle strain, joint injury, cardiovascular events, or other physical injury. I voluntarily choose to participate and accept full responsibility for any risks, injuries, or damages that may occur.

I confirm that I am physically capable of participating in fitness activities and that I have consulted a physician if I have any medical conditions, injuries, or concerns.

CPENAX FIT coaching services are provided for educational and informational purposes only and do not constitute medical advice, diagnosis, or treatment.
Release of Liability
I release and hold harmless CPENAX FIT, its owners, coaches, employees, and affiliates from any and all liability, claims, demands, or causes of action arising out of my participation in CPENAX FIT services.
